The amount of time or commitment is determined by your availability. The EEFHT is a Blended Salary Model (BSM) Family Health Team where total compensation is based on multiple components including base salary, capitation payments, and shadow billing. What you will be doing:
  • Effectively manage the primary care needs of a roster of patients with the help of a multidisciplinary team.
  • Be involved with health promotion activities with the aim of optimizing the health of people, families, communities, and populations;
  • Function within their scope of practice;
  • Demonstrate and maintain competence in clinical practice in health assessment, diagnosis, and therapeutics; and
  • Ensuring standards of patient care are maintained according to accepted medical standards and company policy.
What you will need to be considered:
  • Medical license and current registration with the College of Physicians and Surgeons of Ontario in Family Medicine;
  • Ability to provide proof of professional liability protection (PLP);
  • Knowledge of the Health care system, social determinants of health and community resources;
  • Demonstrated commitment to client-centered, community-based care;
  • Experience working in community primary care settings;
  • Experience using electronic medical records (PS Suites an asset);
  • Strong interpersonal communication skills;
  • Demonstrated organizational and time management skills;
  • Demonstrated experience in program planning, research and evaluation;
  • Ability to work collaboratively within and across inter-disciplinary teams;
  • Ability to work effectively with people of various cultural, age and economic backgrounds; and
