Reporting to the Director, East End Family Health Team, the Registered Practical Nurse (RPN) will be responsible for the provision of primary health services in accordance with the Standards of Practice of the College of Nurses of Ontario (CNO) for Registered Practical Nurses. The incumbent will be responsible for the assessing, collaborative care planning, implementation of the plan of care and evaluation and report any changes in the patient’s condition to appropriate team members.
What you will be doing:
- Assess patient’s health needs (physical, emotional, psychological and spiritual) through nursing health history and physical examination;
- Analyze health assessment findings and reporting any changes to the primary care provider;
- Develop and discuss appropriate individualized care plans with the patient/family/care giver based on best practice in collaboration with the EEFHT;
- Provide primary health care services in accordance with the Standards of Practice, CNO, including but not limited to initial health history, assessment of vital signs, heights/weights, diagnostic sample collection, immunization, well baby visits, administration of medications (as per medical directives and direct orders), foot care etc.;
- Document assessments, treatments provided and evaluations in the electronic medical records system as well as referrals and follow-up as necessary;
- Provide health teaching to the patient whose coping mechanisms and support systems are generally effective;
- Inform patients of availability of health resources and assisting them to identify and use those appropriate for their own medical circumstance;
- Ensure medical supplies and equipment are available in all examining rooms on a daily basis.
- Communicate effectively with health care team members to create a cohesive team and provide seamless services to patients;
- Maintain and develop professional competence through ongoing professional development; and
- Develop and coordinate student placements.

What you will need to be considered:
- Two (2) year diploma in Practical Nursing
- Minimum of two (2) years’ experience in the nursing profession. Preferably an acute care setting (medical/surgical departments);
- Current registration in good standing with the College of Nurses of Ontario;
- Current member with the RPNAO;
- Ability to provide evidence of liability insurance; and
- CPR Level HCP Certification is required.
